DOPE, INC. IS RECALLING 42,540 COMBINATION CORNER AND BUMPER LAMP ASSEMBLIES OF VARIOUS PART NUMBERS SOLD FOR USE AS AFTERMARKET EQUIPMENT FOR VARIOUS PASSENGER VEHICLES. THESE HEADLAMPS FAIL TO CONFORM TO THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 108, "LAMPS, REFLECTIVE DEVICES, AND ASSOCIATED EQUIPMENT." THESE LAMPS DO NOT CONTAIN THE REQUIRED AMBER SIDE REFLECTORS.
Consequence: DECREASED LIGHTING VISIBILITY MAY RESULT IN A VEHICLE CRASH.

We rented a Class C RV based on a Ford E-Series chassis from El Monte RV. During our trip, we experienced two dangerous tire tread separations. The first incident occurred on the third day of our trip on the highway. The right rear tire's tread detached while driving at moderate speed (~55–60 mph), causing the tire to explode. The violent separation destroyed the wheel well area and shook the entire vehicle violently. Our RV contained a [XXX] child, and we were forced to stop on the shoulder in a dangerous situation. The second tire separation happened two days later, again involving a rear tire. Fortunately, it was a slower separation and didn’t cause structural damage, but it further endangered our family and disrupted our trip. We believe these tires were not in a safe condition when the vehicle was rented to us. One of the separated tires showed severe dry rot and age. We had to pay for a replacement tire out-of-pocket during the first incident. The RV company has not responded to our compensation request. These issues could have resulted in serious injury or fatality, and we are filing this report so NHTSA can investigate if this vehicle or others of its kind have similar tire issues. FORD IS RECALLING 225,000 MY 1992-2003 ECONOLINE, MY 1993-1995 TAURUS SHO, MY 1992-1998 CROWN VICTORIA, MY 1993 BRONCO, F-SERIES, MY 1995-1997 F-SERIES SUPER DUTY STRIPPED CHASSIS, MY 1992-1998 MERCURY GRAND MARQUIS, AND MY 1992-1995 LINCOLN TOWN CAR VEHICLES. THE AFFECTED VEHICLES WERE PREVIOUSLY REPAIRED USING A WIRING HARNESS THAT IS NOT COMPATIBLE WITH THE VEHICLE CIRCUIT POLARITY.
Consequence: AS A RESULT, THE FUSE IS LOCATED IN THE OUTPUT CIRCUIT RATHER THAN IN THE INTENDED INPUT POWER FEED CIRCUIT, AND MAY NOT OFFER THE INTENDED PROTECTION IN THE EVENT OF AN ELECTRICAL SHORT TO GROUND.
Remedy: DEALERS WILL INSTALL A NEW WIRING HARNESS, OR REPLACE THE MATING ELECTRICAL COMPONENT. THE RECALL BEGAN ON FEBRUARY 18, 2008. OWNERS MAY CONTACT FORD AT 1-800-392-3673.

VEHICLE DESCRIPTION: BI-FUEL CNG PICKUP TRUCKS AND BI-FUEL CNG VANS, MODIFIED BY CFI CONTROL SYSTEMS, INC., FAIL TO CONFORM TO THE REQUIREMENTS OF FMVSS NO. 303, "FUEL SYSTEM INTEGRITY OF COMPRESSED NATURAL GAS VEHICLES." THE LABEL AFFIXED TO THE FUEL FILLER DOOR BY THE VEHICLE MODIFIER CONTAINS THE INSPECTION DATE AND EXPIRATION DATE OF THE CNG TANK. HOWEVER, IT DOES NOT CONTAIN THE STATEMENT "SEE INSTRUCTIONS ON THE FUEL CONTAINER FOR INSPECTION AND SERVICE LIFE."
Consequence: THESE VEHICLES DO NOT COMPLY WITH THE REQUIREMENTS.
Remedy: OWNERS WILL BE PROVIDED A LABEL WITH THE REQUIRED STATEMENT, ALONG WITH INSTRUCTIONS FOR AFFIXING THE LABEL AT THE FUEL FILLER DOOR. OWNER NOTIFICATION BEGAN NOVEMBER 2, 2000. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT FORD AT 1-800-392-3673.

VEHICLE DESCRIPTION: CUTAWAYS AND STRIPPED CHASSIS WITH SINGLE REAR WHEELS. THE LUG NUTS CAN POTENTIALLY BOTTOM OUT ON A SHOULDER ON THE STUDS.
Consequence: IF THIS OCCURS, THERE CAN BE AN INADEQUATE CLAMP LOAD TO ATTACH THE WHEEL TO THE AXLE, RESULTING IN NOISE, DAMAGE TO THE WHEEL AND HUB, OR IN EXTREME CASES, LOSS OF A REAR WHEEL WHILE THE VEHICLE IS BEING DRIVEN.
Remedy: DEALERS WILL REPLACE THE REAR WHEEL ATTACHING STUDS.

TL* THE CONTACT OWNS A 2000 FORD E-350. WHILE DRIVING 70 MPH, THE VEHICLE ACCELERATED WITHOUT WARNING. THE BRAKE PEDAL WAS DEPRESSED, BUT THE VEHICLE FAILED TO DECELERATE. THE CONTACT PLACED THE GEAR SHIFTER INTO NEUTRAL TO STOP THE VEHICLE. THE CONTACT COASTED THE VEHICLE OVER TO THE SIDE OF THE ROAD AND POWERED OFF THE ENGINE TWICE. WHEN THE VEHICLE WAS RESTARTED, IT OPERATED NORMALLY AND WAS DRIVEN TO THE CONTACT'S RESIDENCE. IN ADDITION, WHILE DEPRESSING THE BRAKE PEDAL, SMOKE APPEARED FROM THE BRAKE PEDAL. THE VEHICLE WAS NOT TAKEN TO A DEALER OR AN INDEPENDENT MECHANIC FOR DIAGNOSTIC TESTING. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 169,000.

THE FUEL TANK ON THIS FORD E150 VAN LEAKS FUEL DURING FILL UPS. THE FORD DEALER (PACIFICO FORD IN BROOMALL, PA) STATES THAT THE VEHICLE NEEDS A NEW GAS TANK. THE FUEL TANK DEVELOPED CRACKS AND PIN HOLES AT THE TOP OF THE TANK. RESEARCH ONLINE STATES THAT THE SAME PROBLEM OCCURED WITH OTHER FORD TRUCK AND VANS AND A RECALL WAS ISSUED. THE RECALL, NAMELY 01V114000 COVERS FORD VANS MANUFACTURED BETWEEN 04/09/1996- 11/14/2000. MY VEHICLE CERTIFICATE OF ORIGIN FROM FOR IS DATED AUGUST 28, 2000 AND THEREFORE PROVES THAT THE VEHICLE WAS MANUFACTURED WITHIN THE SPECIFIED DATES. THIS VEHICLE IS USED AS A WORK TRUCK, SO IT OPERATES NEAR THE GROSS VEHICLE WEIGHT. DUE TO A LACK OF INSULATORS SEPERATING THE TANK AND STEEL CROSS MEMBERS, THE CONSTANT RUBBING CAUSED THE DETERIATION OF THE FUEL TANK.

ONE OF OUR VANS' BCT JB42 FRONT TIRE BLEW UP LEAVING FROM A PARKED AREA WITHOUT GOING OVER ANY SHARP OBJECTS OR CURVES. FORTUNATELY NO ONE WAS HURT BUT IT COULD HAVE CAUSED A HUGE ACCIDENT ON THE ROAD. I CHECKED FOR RECALL ON THIS TIRE BUT IT WAS NOT... I WILL NOW HAVE TO REPLACE 12 OF THESE TIRES ON MY OTHER VEHICLES BECAUSE I CAN'T TAKE ANY MORE CHANCES WITH THESE TIRES ALTHOUGH THEY ARE IN DECENT CONDITIONS... MY FUEL TANK INTERIOR FLAKED SO SEVERELY IT CAUSED THE ENGINE TO BUCK VIOLENTLY AND DRAMATICALLY LOOSE POWER. THIS WAS CAUSED BY THE ALMOST COMPLETE BLOCKAGE OF THE FUEL TANK PICKUP SCREEN/FILTER. SINCE THAT TIME I HAVE HAD TO DISASSEMBLE AND THOROUGHLY CLEAN THE FUEL PICKUP OF THE FLAKES 4 TIMES INCLUDING TODAY. EACH TIME THE PICKUP WAS PACKED WITH SILVER FLAKES FROM THE DETERIORATING LINING OF THE FUEL TANK. THE PROBLEM OCCURRED EACH TIME WHILE TRAILERING MY RV DOWN THE INTERSTATE. IT IS EXTREMELY SCARY AND THE ALMOST COMPLETE LOSS OF POWER MAKES IT VERY DIFFICULT TO GET TO THE NEXT EXIT. THE PROBLEM IS SO PERSISTENT THAT I HAVE PUT A DOOR IN THE FLOOR OF MY VAN SO THAT I NO LONGER HAVE TO DROP THE TANK FOR ACCESS. I PURCHASED THIS FORD E-350 DIESEL NEW IN 2000. *TR
