Consequence: An inflator explosion may result in sharp metal fragments striking the driver or other occupants, resulting in serious injury or death.
Remedy: GM will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. Interim letters notifying owners of the safety risk were mailed on February 24, 2021. Second notification letters will be mailed once the remedy is available. Owner notification letters were mailed on May 11, 2021. The contact owns a 2009 Chevrolet Silverado 2500. The contact stated while driving 30 mph, when an unknown warning light was illuminated. The contact called an independent mechanic who informed the wiring harness was melted and needed to be replaced. The vehicle was not diagnosed or repaired. A dealer was not contacted. Upon investigation, the contact associated the failure with NHTSA Campaign Number: 14V407000 (Electrical System), however, the VIN was not included. The manufacturer had been informed of the failure. The failure mileage was approximately 195,000.
The contact owned a 2009 Chevrolet SIlverado. The contact stated that while driving at 50 MPH, the contact drifted off the highway which caused his vehicle to flip over three times and land right side up. The contact was able to escape the vehicle without major injury. The air bags did not deploy during the crash. The authorities arrived at the scene and the contact was examined by paramedics. A police report was filed. The vehicle was towed to an independent tow yard where it was destroyed. Later that evening, the contact went to the emergency room where he was treated for a broken left rib. The dealer nor the manufacturer had yet to be notified of the failure. General Motors LLC (GM) is recalling certain 2009-2014 Chevrolet Silverado HD and GMC Sierra HD, 2009 GMC Topkick 5000, 6000, 7000 and 8000 Series and Isuzu F Series, and 2009-2018 Chevrolet Express, and GMC Savana vehicles, equipped with certain Plastic-Handle Fire Extinguishers or Push Button 'Pindicator' Fire Extinguishers. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Consequence: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.
Remedy: GM will notify owners, instructing them to contact Kidde to get a replacement fire extinguisher, free of charge. The recall began June 11, 2018. General Motors LLC (GM) is recalling certain model year 2007-2011 Chevrolet Silverado HD 2500/3500 trucks manufactured January 18, 2007, to June 22, 2011, and GMC Sierra HD 2500/3500 trucks manufactured January 22, 2007, to June 21, 2011, and equipped with an auxiliary battery. Owners may unknowingly draw too much current through the vehicle's trailer harness resulting in the melting of fusible link for the auxiliary battery.
Consequence: If the fusible link melts it may come into contact with adjacent components, damage the electrical center cover, the nearby wiring harness conduit and other vehicle components, increasing the risk of a fire.
Remedy: GM will notify owners and dealers will replace the jumper harness with one that includes a 40 amp inline fuse, and provide the owner with supplemental Owner Manual information, free of charge. A RECALL WAS IMPLEMENTED IN 2008 TO ADD A FUSE TO THE CONTROL CIRCUIT HARNESS TO ADDRESS THE POTENTIAL CONSEQUENCES OF A PRINTED CIRCUIT BOARD (PCB) ELECTRICAL SHORT. HOWEVER, THERE HAVE BEEN NEW REPORTS OF THERMAL INCIDENTS ON HWFS MODULES AFTER THIS IMPROVEMENT WAS INSTALLED. THESE INCIDENTS RESULTED FROM A NEW FAILURE MODE ATTRIBUTED TO THE DEVICE'S THERMAL PROTECTION FEATURE. THE SIGNIFICANCE VARIES FROM MINOR DISTORTION TO CONSIDERABLE MELTING OF THE PLASTIC AROUND THE HWFS FLUID CHAMBER.
Consequence: IT IS POSSIBLE FOR THE HEATED WASHER MODULE TO IGNITE AND A FIRE MAY OCCUR.
Remedy: DEALERS WILL PERMANENTLY DISABLE AND REMOVE THE HEATED WASHER FLUID MODULE. AN UPDATED PAGE FOR THE OWNER MANUAL WILL BE PROVIDED AND INSERTED IN THE OWNER MANUAL TO DOCUMENT THAT THE FEATURE HAS BEEN PERMANENTLY DISABLED AND REMOVED FROM THE VEHICLE. Takata recall. The contact owns a 2009 Chevrolet Silverado 3500. The contact received a recall notice for NHTSA Campaign Number 21V050000 (Air Bags) however, upon contacting the dealer, he was informed that the remedy may cause the dashboard to crack. The contact was then informed that he had to sign a waiver to ensure that the dealer would not be held responsible for any damages to vehicle. The manufacturer was then notified of the issue and confirmed that the dealer would not be held responsible for any cracks or damages to the dashboard. The contact declined to sign the waiver and the vehicle was not repaired. The contact stated that the manufacturer should cover the cost of the dashboard because of the recall. TL* THE CONTACT OWNS A 2009 CHEVROLET SILVERADO 3500. WHILE DRIVING 35 MPH, THE CONTACT HEARD A NOISE THAT SOUNDED LIKE A GUNSHOT. THE CONTACT NOTICED THAT BOTH SEAT BELT RETRACTORS DEPLOYED WITHOUT WARNING. THE CONTACT COASTED THE VEHICLE OVER TO THE SIDE OF THE ROAD. THE SEAT BELT SECURED THE CONTACT, BUT HE WAS ABLE TO RETRACT THE SEAT BELT. THE CONTACT CALLED CHAMPION CHEVROLET OF HOWELL (5000 E GRAND RIVER AVE, HOWELL, MI 48843, (866) 354-3325) AND WAS INFORMED THAT THERE WAS NO TECHNICAL SERVICE BULLETIN OR RECALLS ON THE VEHICLE. THE VEHICLE WAS NOT TAKEN TO A DEALER OR INDEPENDENT MECHANIC AND WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 233,000.
THE DASHBOARD IS CRACKING IN SEVERAL PLACES AND I'M AFRAID THAT IF THE AIRBAG IS DEPLOYED IT MAY SEND PART OF THE DASH INTO THE PASSENGER.
WHILE DRIVING THE VEHICLE, THE ENGINE SUFFERED COMPLETE FAILURE DUE TO A SHEARED CAMSHAFT PIN. THIS RESULTED IN LOSS OF CONTROL OF THE VEHICLE SINCE IT WOULD NOT RESTART. I NARROWLY MISSED HAVING AN ACCIDENT BECAUSE THIS HAPPENED ON A BUSY MAIN STREET. I FEEL THIS TO BE A VERY SERIOUS SAFETY ISSUE TO WHICH A RECALL SHOULD BE REQUIRED.

THE DASHBOARD IS CRACKED ABOVE THE AIRBAG AND ABOVE THE RADIO. THE VEHICLE HAS LESS THAN 60,000 MILES AND ACCORDING TO CHEVROLET IS OUT OF WARRANTY. THIS IS A SAFETY ISSUE DUE TO THE FACT THAT IF I AM IN A WRECK THE DASH COULD PUNCTURE THE AIRBAG OR EVEN ANOTHER PASSENGER.
2009 SILVERADO CRACKED PASSENGER SIDE DASH BOARD HAD IT REPLACED BY GMC DEALER AND IT CRACKED AGAIN SAME PLACE AS THE FIRST CRACK. TRUCK HAS LESS THEN 77 THOUSAND MILES.
THE DASH PANELS ON THE TOP OF THE DASH BOARD HAVE COMPLETELY BROKEN I FEEL THAT IF THE PASSENGER AIRBAG WERE TO DEPLOY IT WOULD BE A SAFETY ISSUE. PIECES OF THE DASH COULD POSSIBLY BECOME A HAZARD AND INJURE MYSELF OR MY PASSENGERS THIS NEEDS TO BE ADDRESSED THIS IS A VERY COMMON ISSUE AMONG MANY CHEVROLET VEHICLES

CRACKING IDENTIFIED NEAR THE PASSENGER SIDE AIRBAG ON THE DASH OF THE TRUCK. DISCOVERED WHEN REPLACING TRUCK WINDSHIELD. CRACKING ALSO SEEN ON OPPOSITE SIDE OF DASH.
SEVERAL CRACKS IN MY DASH (4-5). ONE IS AT THE CORNER OF THE PASSENGER AIRBAG. THE DASH IS SO BRITTLE IT CRACKS FROM JUST CLOSING THE DOOR. I'M AFRAID WHAT MIGHT HAPPEN IF I'M IN AN ACCIDENT AND BROKEN DASH PIECES GO FLYING THROUGHOUT THE CAB. THIS IS NO DOUBT A SAFETY CONCERN! NOT TO MENTION AN EYE SORE AND RESALE KILLER. THIS STARTED A FEW YEARS AGO AND CONTINUES TO GET WORSE.
