Component: FUEL SYSTEM, GASOLINE:DELIVERY:HOSES, LINES/PIPING, AND FITTINGS
CERTAIN R&B, INC. CENTRAL MULTIPORT FUEL INJECTION (CMFI) FUEL RETURN LINES, MODELS HELP 55163, NAPA BALKAMP 700-3270, AND THE CONNECTION 2560001, SOLD FOR USE AS REPLACEMENT EQUIPMENT FOR THE VEHICLES LISTED ABOVE. THE FUEL RETURN LINE HAS A MECHANICAL BLEED VALVE WHICH WAS NOT INSTALLED AT THE PROPER TORQUE.
Consequence: A FUEL LEAK CAN OCCUR AT THE VALVE, POSSIBLY RESULTING IN A FIRE.
Remedy: R&B, INC. WILL NOTIFY OWNERS AND AT THE CUSTOMERS REQUEST, EITHER REPLACE THE FUEL LINE OR OFFER A FULL REFUND. THE RECALL BEGAN ON MAY 4, 2006. OWNERS MAY CONTACT R&B, INC. AT 215-712-5330.
Component: EXTERIOR LIGHTING
CERTAIN PRO-A MOTORS CORNER LAMPS, TURN SIGNALS, AND HEADLIGHTS SOLD AS REPLACEMENT LAMPS FOR USE ON CERTAIN PASSENGER VEHICLES LISTED ABOVE. SOME COMBINATION LAMPS THAT ARE NOT EQUIPPED WITH AMBER SIDE REFLECTORS FAIL TO CONFORM TO FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 108, LAMPS, REFLECTIVE DEVICES, AND ASSOCIATED EQUIPMENT.
Consequence: WITHOUT THE AMBER REFLECTORS, THE VEHICLE WILL BE POORLY ILLUMINATED, POSSIBLY RESULTING IN A VEHICLE CRASH WITHOUT WARNING.
Remedy: PRO-A MOTORS WILL NOTIFY OWNERS AND OFFER TO REPURCHASE THE LAMPS. THE RECALL BEGAN ON APRIL 3, 2006. OWNERS MAY CONTACT PRO-A MOTORS AT 323-838-2988.
TL* THE CONTACT OWNS A 1993 GMC JIMMY. WHILE DRIVING IN REVERSE AT 20 MPH, THE VEHICLE BECAME UNCONTROLLABLE AND WAS UNABLE TO BE STOPPED BY DEPRESSING THE BRAKE PEDAL. THE FAILURE RECURRED THREE TIMES. THE MANUFACTURER WAS NOTIFIED, BUT THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 99,426.

VEHICLE DESCRIPTION: CERTAIN 4-WHEEL DRIVE PICKUP TRUCKS AND SPORT UTILITY VEHICLES EQUIPPED WITH ABS (ANTILOCK BRAKING SYSTEM) MANUFACTURED FROM SEPTEMBER 1989 THROUGH AUGUST 1996. UNDER CERTAIN DRIVING CONDITIONS THE SWITCH WHICH SIGNALS THE ABS SYSTEM WHETHER THE VEHICLE IS IN 2-WHEEL OR 4-WHEEL DRIVE CAN MALFUNCTION CAUSING INCREASED STOPPING DISTANCES DURING ABS STOPS WHILE IN THE TWO-WHEEL DRIVE MODE.
Consequence: IF THIS OCCURRED AT A TIME WHEN MINIMUM STOPPING DISTANCE WAS REQUIRED, A VEHICLE CRASH COULD OCCUR WITHOUT PRIOR WARNING.
Remedy: DEALERS WILL REPLACE OR REPAIR THE 4-WHEEL/2-WHEEL DRIVE SWITCH.
Component: POWER TRAIN:AUTOMATIC TRANSMISSION
SIGNIFICANT WEAR CAN DEVELOP ON THE LOW AND REVERSE CLUTCH PREVENTING THE PARK PAWL FROM ENGAGING. THIS CAN PREVENT A "NO PARK" CONDITION TO OCCUR WHILE THE ENGINE IS RUNNING, CAUSING UNINTENTIONAL MOVEMENT OF THE VEHICLE WITHOUT OPERATOR ACTION. (THE "NO PARK" CONDITION WILL NOT OCCUR WHILE THE ENGINE IS OFF.)
Consequence: UNINTENTIONAL VEHICLE MOVEMENT COULD RESULT IN A VEHICLE ACCIDENT AND INJURY TO PERSONS IN THE PATH OF THE VEHICLE.
Remedy: DEALERS WILL CHECK THE TRANSMISSION FLUID FOR CONTAMINATION WHICH WILL INDICATE IF THE LOW OR REVERSE CLUTCH IS WORN. IF THE TRANSMISSION FLUID SHOWS CONTAMINATION, THE TRANSMISSION WILL BE REPLACED.

THIS IS NOT A SAFETY RECALL IN ACCORDANCE WITH THE SAFETY ACT. HOWEVER, IT IS DEEMED A SAFETY IMPROVEMENT CAMPAIGN BY THE AGENCY. VEHICLE DESCRIPTION: CERTAIN LIGHT DUTY PICKUP TRUCKS, SPORT UTILITY VEHICLES, OR MINIVANS EQUIPPED WITH 3-SENSOR ABS (ANTILOCK BRAKING SYSTEM) EBC4 UNITS.
Consequence: THESE VEHICLES CAN EXPERIENCE EXTENDED STOPPING DISTANCES DURING ABS STOPS ON CERTAIN MULTIPLE SURFACES, INCREASING THE RISK OF A CRASH.
Remedy: DEALERS WILL MODIFY THE COMPUTER PROGRAM WHICH OPERATES THE ABS UNIT ON SOME VEHICLES, OR REPLACE THE COMPUTER ON OTHERS TO IMPROVE THE PERFORMANCE OF THE ABS ON THESE SURFACES.
DT: WHEN DRIVING THE VEHICLE AND PRESSING ON THE BRAKE IT FELT AS THOUGH THEY DIDN'T WORK. VEHICLE WAS TAKEN TO THE REPAIR CENTER, AND WAS TOLD TO UNHOOK THE ANTI-LOCK BRAKES BECAUSE IT WAS EXPENSIVE TO FIX THEM , AND THAT WAS WHAT MOST PEOPLE DID. *AK
FUEL INJECTION WAS REPLACED THREE TIMES. ALSO, CONSUMER HAD A PROBLEM STARTING THE VEHICLE. TO OBTAIN SPEED CONSUMER HAD TO COMPLETELY DEPRESS THE ACCELERATION PEDAL. *AK
BRAKES FAILED ON 4 OCCASIONS. BRAKE PEDAL WENT TO FLOOR WHEN ATTEMPTING TO STOP ABRUPTLY. I DID SOME RESEARCH AND FOUND THAT THIS WAS A PROBLEM WHICH HAD A RECALL ALTHOUGH I DIDN'T OWN THE VEHICLE DURING THAT TIME. I CONTACTED THE PREVIOUS OWNER WHO SAID HE DIDN'T HAVE IT FIXED AND WASN'T SURE IF HE EVER RECEIVED A RECALL NOTICE. I NOTIFIED GMC AND WAS TOLD BY THAT MY VIN WAS NOT FOUND . AM I STUCK WITH THIS PROBLEM? I FOUND OUT IT WOULD COST APPROXIMATELY $800.00 TO FIX! IT IS VERY DANGEROUS. *LA
WHILE DRIVING THE BATTERY INDICATOR STARTED FLASHING. THE CONSUMER PULLED INTO A SERVICE STATION, OPENED THE HOOD AND SAY FLAMES COMING FROM THE ALTERNATOR. THE FIRE WAS EXTINGUISHED AND A NEW ALTERNATOR WAS INSTALLED. *PH *NLM
I WOULD LIKE TO KNOW WHY I HAVE TO CHANGE THE EGR VALVE EVERY SIX MONTHS, AND I ONLY RECEIVE A ONE MONTH WARRANTY ON IT WHEN PURCHASED? WE ARE NOT SURE IF OUR ABS HAS EVER BEEN SERVICED SINCE THE RECALL. I WOULD LIKE TO KNOW HOW WE CAN GET INFORMATION ABOUT IT, AND IF IT HAS NOT, IS IT TOO LATE TO BE SERVICED WITHOUT US PAYING FOR IT? *JB
TREAD SEPERATION IN PHANTOM A/P TIRE THAT WAS MANAUFACTURED WITHIN TIME PERIOD OF NHTSA ACTION #PE02056. SUBSTANTIAL PHYSICAL INJURY TO 72 YEAR OLD DRIVER AND TOTAL LOSS OF VEHICLE. RIMKUS CONSUTLING SERVICES EXPERT GARRET MITCHELL MADE THE FOLLOWING CONCLUSIONS: 1.A DEFECT IN THE LEFT-REAR TIRE ON THE CHEVROLET CAUSED THE TREAD TO SEPARATE FROM THE CARCASS. THE NATURE OF THE FAILURE IS CONSISTENT WITH CONTAMINATION OR IMPROPER VULCANIZATION OF THE TIRE COMPONENTS DURING MANUFACTURING. 2. THE SUDDEN FAILURE OF THE TIRE CAUSED THE DRIVER TO LOSE CONTROL OF THE VEHICLE AND SUBSEQUENTLY COLLIDE WITH A SOLID OBJECT. "THERE WAS A PRESENCE OF POLISHING AND BLUING ON THE RUBBER CARCASS OF THE TIRE [WHICH INDICATED] OVERHEATING OF THE TIRE. . . . IF EVIDENCE OF OVERHEATING IS OBSERVED AROUND THE ENTIRE CIRCUMFERENCE OF THE TIRE, FLEXING OF THE TIRE DUE TO OVERLOADING OR UNDER-INFLATION IS INDICATED. IN THIS CASE, HOWEVER, THE EVIDENCE OF OVERHEATING IS LIMITED TO ONE AREA OF THE TIRE, SPECIFICALLY THE AREA WHERE THE TREAD BEGAN TO SEPARATE FROM THE CARCASS. THIS INDICATES LOCALIZED OVERHEATING DUE TO FRICTION BETWEEN ADJACENT TIRE COMPONENTS WAS TAKING PLACE IN THIS AREA. FURTHERMORE, THE REMAINING TREAD IN THIS AREA IS UNEVENLY WORN, INDICATING A DISCONTINUITY IN THE TIRE STRUCTURE AT THIS LOCATION. THESE OBSERVATIONS INDICATE THE COMPONENTS OF THE TIRE WERE NOT PROPERLY BONDED TOGETHER IN THE AREA WHERE THE TREAD SEPARATION BEGAN."
