After closing the door, it was discovered that all the doors had erroneously locked causing the child to become trapped inside the vehicle until her husband was able to open the door using a second key fob. The cause of the failure was not yet determined. Prior to the failure, the contact indicated that the key fob auto start feature had malfunctioned and failed to start the vehicle. The local dealer was notified of the failure. The manufacturer was not yet contacted. The VIN was not available. The approximate failure mileage was 13,000.

THE CONTACT STATED THAT WHILE ANOTHER PERSON WAS DRIVING THE VEHICLE AT 45 MPH, HE ATTEMPTED TO PASS ANOTHER VEHICLE BUT COLLIDED WITH ANOTHER VEHICLE IN FRONT OF HIM. THE CONTACT STATED THAT THE AIR BAGS DID NOT DEPLOY UPON IMPACT. BOTH THE DRIVER AND CONTACT WERE TRANSPORTED TO THE HOSPITAL VIA AN AMBULANCE. THE CONTACT WAS TREATED FOR A BROKEN NOSE AND A MAJOR BRUISE TO HER LEFT THIGH. THE DRIVER WAS TREATED FOR THREE BROKEN RIBS ON HIS RIGHT SIDE, BRUISES TO THE LEFT SIDE OF HIS HEAD, AND A BROKEN LEFT ARM. A POLICE REPORT WAS FILED AT THE SCENE. THE VEHICLE WAS TOWED TO AN INDEPENDENT MECHANIC WHERE THE WHEREABOUTS OF THE VEHICLE REMAINED UNKNOWN. THE RENTAL COMPANY STATED THAT A CLAIM HAD BEEN FILED WITH THE MANUFACTURER. THE FAILURE MILEAGE WAS APPROXIMATELY 19,000.
