The ABS, BREAK, and Traction Control, suddenly turned on when I was traveling on the freeway in mild traffic. I was traveling at about 60-65 mph. I attempted to use the breaks and could feel a substantial difference in how much and how hard I had to push in order to stop. The problem has not been confirmed by a dealer or service center. The vehicle was inspected by my brother in law who works on cars frequently. There were no warning symptoms prior to the problem. The problem first occurred on Saturday November 1st at about 12:30. According to my brother in law, it was the break pump that failed; you can hear air escaping when you push on the break pedal.

Brake booster failed. ABS lights on. Brake light on. Traction control light on. Was sitting at stop with foot gently on brake when lights turned on. Attempted to restart car to clear lights, lights would not turn off after normal restart x3. Brake no longer function correctly, and the factory installed ABS are no longer functioning. Braking distance greatly increased, and no longer as safe to operate.

While driving my Prius on the highway, the ABS and traction control warning light suddenly came on. The brakes started feeling different right away, and it didn’t feel safe to drive. I took it to a Toyota dealership, and they found that the brake booster and pump had failed. I looked online and saw that many other Prius owners had the same issue, so it doesn’t seem like an isolated case.

I purchased my 2015 Prius C in July of 2016. Since then I have consistently had issues with the lock actuator going out on various doors. As follows: 3/12/18 - Driverside 6/13/18 - Passenger Front 8/3/20 - Driverside 8/5/22 - Rear Driverside 12/16/22 - Passenger Front 3/14/23 - Driverside and now in July of 2025 the Driverside actuator is failing, AGAIN. The previous times were covered by my warranty, but that ended in 2023. My car is also kept in a garage. The dealership attempted to blame this issue on the elements, but come on...if that was the case, this would be happening to all sorts of various cars, and it's not. This is an issue for a variety of reasons, one being I am unable to access getting into my vehicle with the door lock that is not working. I currently have to go to my passenger side and crawl over to unlock my driverside door. If I needed to get into my car quickly, I would be out of luck. This is RIDICULOUS as this is the SEVENTH time a lock has gone out - making it about every year and a half that a lock needs to be replaced - and the 4th time for my driverside. How is this not a recall issue???

The vehicle has triggered DTC codes C1256 and C1391, which indicate failure of the brake booster and master cylinder — a problem widely reported by Prius owners. The defect causes decreased brake effectiveness and increased stopping distance. My Prius is just 5,470 miles over Toyota’s 150,000-mile coverage cap, and despite confirming the exact codes, the dealership refused to repair unless Toyota pays first. Toyota Corporate opened Case #[XXX], but has not offered a resolution. This is a safety-critical issue that Toyota acknowledges in internal service bulletins, but they are denying assistance based on a technicality, despite my vehicle having the exact failure their program is designed to address. Failure of brake pump and brake cylinder assembly system, resulting in reduced braking ability and stiff brake pedal. Per Toyota's diagnostic: trouble code C1391 is present, abnormal leak in accumulator, replaced brake ABS actuator assembly, along with brake master cylinder, and bleed the brake hydraulic system. No warning lamps or safety indicators prior to failure. Upon failure (while on freeway going 50+ MPH) the ABS, Brake, and traction control warning lamps illuminated. The failure resulted in no accident or injury, however it did occur while on the freeway going freeway speeds, and resulted in difficulty in braking, which was dangerous. Took Prius into Toyota dealer for diagnostic and repair, was told that part was known to fail in this model of Prius, and had a manufacturer designated extended warranty for 10 years or 150,000 miles. However, despite prius being under 150,000 miles, it passed ten years since purchase in march 2025, meaning I had to pay in full for the repair ($3,631.96). The car gave no indication of needing the repair until the day of the failure, which was on June 18th 2025. If the vehicle had given some prior indication of brake pump system failure or fault I would have had it repaired prior to the warranty expiration, but it did not, leaving me to pay out of pocket for a failed part that Toyota is at fault for making prone to failure.

Brake booster failed while driving down the mountains back from big bear. Nearly wrecked several times, had no idea why my brakes weren't working properly. Go to AutoZone to scam the code, and I got the dreaded ABS failure. Sadly the extended warranty for this known issue from Toyota conveniently expired 2 weeks before the failure. Toyota should be held accountable and pay for my repair, that O did myself. Parts cost me $1800, plus a day off from work I had to take. Atrocious situation, Toyota should be ashamed.

While driving my 2015 Toyota Prius, the brake pedal became extremely stiff and hard to press. I was on the way to the dog park with my dog in the back seat when I had to press down very hard just to slow the car at a traffic light. It felt like I was stepping on a rock. The brakes barely engaged, and I had to use far more force than normal to get the car to stop in time. When I hit the brakes, they finally caught hard and suddenly and my dog went flying off the back seat and landed on the floor. She’s fine physically, but now she’s terrified of getting in the car at all. It was scary for both of us and felt totally unsafe. If I had been going just a little faster or if someone was walking across that intersection, it could’ve ended badly. I later found out this issue is tied to a known defect with the brake booster and accumulator system, specifically related to code C1391. Toyota even acknowledged this problem in a Customer Support Program (CSP ZJB/ZKK), but they refuse to help because my car is slightly over the mileage limit. That doesn’t change the fact that this is a serious, dangerous failure that affects the driver’s ability to stop safely. Toyota should be forced to take responsibility for this failure before someone gets seriously hurt.

Toyota Hybrid Brake Problems 2015 Toyota Prius I took to my dealership Jim Ellis Toyota in McDonough GA. I was told that my brake master cylinder & brake booster pumps vehicle has failed. I experienced my brakes locking up, causing the vehicle to unexpectedly lunge forward or failing to engage when the brake pedal is depressed. On [XXX] my ABS, Brake, Traction lights came on out of nowhere. I tried taking it to my dealership in April to Jim Ellis, Toyota of McDonough for the lights, the one lights being on. I took it there around 8 o’clock in the morning and then there was a very rude and disrespectful service person by the name of Victoria who told me that they would not be able to see me and told me that I needed to leave because I didn’t I have an appointment. This is a Dealership that I purchased this car phone in 2018 and I have been taking my car to Jim Ellis Toyota of McDonough since 2018 to get regular oil changes and other maintenance for my car. Such as things my brakes replace my front and the back brakes have my battery please and come several times out of the year to get a oil change because I have part of what they consider a lifetime oil change on my car. Also, I had a Warranty whole time that I have been taking my car to the dealership they never once brought to my attention that there was a known recall for the brakes booster. It wasn’t until I took my car to dealership on Friday 5/30/25 that I was advised that this was a known issue for 2010-2015 Toyota Prius Jim Ellis dealership knew about the problem and failed to make repairs when I spoke with general manager Lorenzo Smith he advised that past the recall timeframe and my warranty has expired. Jim Ellis dealership knew their were known issues and hid the issue I refuse to write the wrong by making the repairs to my vehicle. They’re trying to charge me over $3200 in repairs when I spoke with the general manager, he advised me that they didn’t make the repairs because my vehicle vin number. I own a 2015 Toyota prius, purchased new and put in service December 4th, 2014. Car is well maintained. On May 10th, 2025 The Brake booster pump and assembly failed. Has internal leak. I had the dealer confirm this. Lights on the dash, computer code c1391 ( abnormal leak in accumulator, Rec replacement of brake booster and pump) and brake booster pump is pumping every 3 seconds from internal leak. Brakes are now at 50%. Soft. The Pads and shoes are perfectly fine. It’s the brake booster pump assembly. Toyota issued instead of a recall, which would have been the right thing to due considering its long history of these over multiple years failing, just a customer service letter. 10 years or 150,000 miles, which ever comes first. Mine failed at 10 1/2 years. Now I am stuck with a $3,000 bill which I can not afford, to replace the Brake booster pump and assembly. Toyota completely knows of the issue and side stepped having to issue a recall on this very well know problem.

Left our house and as we were driving out of our neighborhood the ABS and Brake lights came on. We looked it up in our book and couldn't find anything concrete to go on. Drove straight to Auto Zone and their test came back most likely Brake booster or master cylinder. While looking it up, we read several stories people had posted saying this was a defect with 2012 - 2015 Toyota Prius' and to write to you and tell of the incident. I put my Vin # in and it doesn't show any recalls about this. Any feedback would be helpful. Thank you!

Car feels like it lunges forward while braking over bumps. I have learned this is a known manufacture defect with the Brake Booster and Brake Actuator which Toyota is refusing to replace outside of 10 years from manufacture date. This is a critical safety item and failure can result in loss of brake power. Most owners do not know this part even exists until failure has happened already.

However I purchased my 2015 Prius used that had over 100 K already never received no notice from manufacturer or the dealer I bought the car from this is not fair and if Toyota is putting in faulty brake boosters, they should be replaced for free regardless of the mileage because I didn’t break the booster It’s the brake booster that was originally installed by the manufacturer! The ABS and traction light appeared on the dash while driving on the freeway. I attempted to brake, while merging to exit but the car brake system did not respond. Luckily there were no cars close to us. Figured it could be a brake fluid problem so I added more fluid and it went away for a bit. It’s come back again and my car feels unsafe to drive. The brake pedal feels stiff or sometimes mushy like dough. I am waiting to get a diagnosis on my car but from what I’ve read and what other prius owners are reporting, it appears to be the “break actuator and break booster pump” that is failing to work properly. Wondering if any other Prius owners have filed a complaint.

I was driving down the road when I noticed my brakes were not working. I had to push a lot harder on the pedal to slow the car down luckily, I did not crash into the car in front of me. I pulled over to see numerous lights on the dash such as traction, abs, and brakes. I had the car towed to Toyota where they quoted me almost $3000 to fix the brake accumulator and booster assembly. They said my car is outside the extended warranty of this problem. My car is a 2015 Prius with 154k miles. I found online that these things fail commonly on this generation of Prius. It's horrible for this company to charge us basically the cost of the car to fix the brakes.

Brake Booster and Master Cylinder failed. Traction Control and ABS lights came on while driving. Upon inspection there is an issue with the brake booster and master cylinder. This is an issue that Toyota is aware of and were repairing under a customer service campaign. They are aware that these parts malfunction and instead of recalling them, they set expiration dates for free replacement. The car must be experiencing an issue and throw the correct code within the campaign window to be eligible for complimentary repair. The brakes failing in this way could cause great harm to me as a driver and others on the road making it imperative that I pay thousands in repair cause to ensure everyone's safety, though that is toyota's job.

The headlight bulbs keep going out. I have replaced the bulbs at least 2-3 times a year for the past few years. I have asked dealers about this and they tell me there is no recall or tsb. This is a hazard to night driving because of decreased illumination, especially in dark roads with not enough lighting. Please recall/fix this issue!!

My small battery died and it is located on passenger side, inside, underneath tail light. There us no key entry. One must crawl to back if car and dismantle many pieces to access release. I had heavy 5 gallon jugs of water in back. There is no trunk release and no key entry. The only way to jump car is at fuse box under hood. Car needs a button inside, ideally up front.

On the morning of December 11th, 2024, the brake system warning light (ABS, yellow) illuminated on the dashboard along with the red BRAKE indicator and the yellow Vehicle Stability Control (VSC) system light. I consulted a mechanic who performed a computer diagnostic and informed me that the ABS module booster had failed. He said that will order the part to change it but meanwhile he advised me to drive more cautiously and maintain a greater distance from the vehicle ahead, allowing extra space for my car to stop due to the increased braking distance caused by the failed ABS module booster. Later that day, at approximately 5:30 PM, I was driving home in heavy traffic, with speeds not exceeding 40 miles per hour. The vehicle in front of me began to decelerate abruptly. When I applied firm pressure to the brake pedal to slow down my car, it lost all control. The vehicle began to skid, with the amplitude of the skid increasing progressively. It struck the highway guardrail on the left, changed trajectory, crossed all four lanes to the right uncontrollably, and ultimately collided with the highway guardrail on the right side.

There is a known brake booster issue with the Toyota Prius. They extended the coverage to 150,000 miles. My car is at 127,000 miles and they said the repair isn't under warranty and will cost over $3,000 to fix. There was no pre-warning lights or information received about this repair notice. I only noticed this when the lights came on and our ABS lights went out. When I took it in they said I missed the deadline by 5 days, that I would have had to let them know by 11/20/2024. Which is insane for such a huge issue when I am well under the mileage coverage.

The clear coat on the roof of my 2015 Prius began to peel little by little starting around 2021. What started out as small patches has now become large patches on various parts of the roof. The clearcoat is also beginning to peel right below the windows on the passenger side, close to where the door panel and the window meet. It's unsightly and I have not had this happen to any of my other vehicles in the past. I've done nothing different in the handling/washing/parking of the vehicle compared to my previous cars.

As many others have reported, the Brake Accumulator has failed. The symptoms began at 100,000 miles. When reported to the dealership, was brushed off as not a safety concern. It is, indeed, a major safety concern. My partner had the ABS lights come on and brakes freeze up while driving on a fast and congested highway. Furthermore, getting this repaired is nearly impossible. The only Toyota dealership in town is estimated a $5k price. And since independent shops often cannot perform this repair, there is no comparison possible. They simply push to buy a new vehicle instead of standing by one that should be operating normally. The dealership is Miracle Toyota of North Augusta SC. Consistently does not support the consumers concerns and simply pushes repurchase as the only reasonable avenue. This is a known issue for Toyota that has stranded many of us without the ability to keep this car on the road. Please resolve with reasonable repair options due to faulty design.

My vehicle has an ongoing issue with the headlights not being bright enough to drive at night with. I have replaced the headlights more than 5 times now over the course of a year, each time with different brands including Toyota OEM headlights. Each time, they are still not sufficiently bright enough to see with at night. This has led to 3 collisions with curbs and objects which caused damage to my vehicle, because I could not see the objects while having to drive at night. I have brought the issue up to Toyota and they said they would not do anything about it. I brought it in for service to Toyota and they tried to charge me $200 to even look at it, money which I do not have. I am forced to continue driving at night for work and fear I will get into a more serious crash or hit a pedestrian as the headlights aren't bright enough to see safely at night. The photo I have included shows a gash in the tire and dent in the rim where it hit the curb. The issue seems to be the headlight wiring or connectors. When I replace the headlight bulbs, they are a bit brighter initially. Thus initial brightness only lasts about a week, and it's still not sufficiently bright to drive safely. They then dim even further to the point where it's like driving without headlights at all. Again this has led to 3 collisions at this point since I'm required to drive at night and don't have the money to fix it. Toyota refused a good faith repair and did not acknowledge there was an issue, however if you do a web search for "Prius headlights too dim" it seems to be a major issue with many other Prius owners. It should be noted that even the high beam headlights are not bright enough to sufficiently see with at night.

My brake booster is failing and has been confirmed with Certified Toyota Master Mechanics at Toyota Dealerships that it is failing. Toyota has a CSP Called ZJB to replace these once the brake booster fails. My car has been identified as one of the cars with a faulty brake booster and I’m terrified to continue to drive it as it has to throw a code and break before they will change the faulty part. I want them to fix it before it fails and someone gets hurt. What do I do? There are a lot of other complaints about this but Toyota is not fixing them until they break. This is a huge safety issue.

When I parked my car and stopped the engine after driving for 90 min/62 miles on June 22, 2023, I found that I couldn't lock my car's door, through my remote, door handle or the switch inside the door. Upon further examination, I saw complete loss of the power: no light, no audio, no other accessory power, no start of engine. Foot press on the brake feels heavy/less responsive. One hr after the incident I went back (because I had some business to attend) to the car, everything looked normal (with full functions recovered) as if nothing happened before. 6 hrs later the car still behaved fine. I measured 12-v battery and it registered 12.3 v (I bought the car new 8 years ago and I haven't changed the battery yet). I noticed that my the clock in the car lost 10 mins (behind correct time) and I concluded the complete loss of battery power lasted about 10 mins. By the time of this report, I have started the engine 5 times without any problem but this loss of power gravely concerns me. I saw recalls for similar problems affecting similar prius models/years however my car is not on the list.

1. Brake accumulator pump is running too often, indicating that there is a pressure leak in the braking system. 2. As designed the pump will run only on occasion to charge the booster fully. This pump runs a lot, whether the car is running, or not, whether the brakes are applied, or not, and regardless of braking rigorously or gently. 3. Occasionally, on start up, brake pedal goes to the floor as the accumulator has lost all charge. 3. No brake warning lights on dash, yet. 4. No service codes known, yet. Toyota had a customer support program to address this issue, extending warranty to 10 years or 150,000 miles. I am at 160,000, 8 years. The problem has been occurring for a while, but I was not aware as it was my wife's car and driven solely by her. I was told today by a Toyota resolution specialist that no help would be forthcoming, and to hope that NHTSA issues a recall wherein my problem with the braking system would then be addressed. This seems problematic for many reasons, not the least of which is that it is regarding BRAKES, an integral part of the safety equipment to ensure our highways are without danger or have no known issues that have not been resolved by a car manufacturer. My Toyota case number is 230426001055. This 'complaint' is to alert this agency that there is a problem with my vehicle, and others, known by Toyota, as evidenced by past Customer Support Program, and as substantiated today by service rep at Family Toyota of Burleson, and there is no recompense available for the $3,000.00 repair required to ensure my vehicle safely travels our nations highways. While I understand that a car cannot be warrantied indefinitely, an eight year old car that has been meticulously maintained with a serious safety time bomb should not fall upon the owner entirely, nor should it be the responsibility of your agency to force the hand of the automaker. Since that is there stance, I appeal to you to respond seriously and help me try to remedy.

I bought this car brand new and have had it serviced regularly at a Greensboro Nc Toyota dealership. It has 127,000 miles on it. I starting hearing a grinding whirring noice a lot lately and determined it is a brake actuator/brake booster. Took it to the dealership and they confirmed it to be a brake booster going bad. Engages at least 4 times a minute. There is a customer support program for this repair from Toyota ZJB and covers the part up to 10 years or 150,000 miles. I am in the covered period but it has to break first before they will cover it even though they confirmed it is going bad. It has to “throw a code” before Toyota will pay. I reached out toToyota and they reached out to the dealership which is Greensboro Toyota in Greensboro, NC and their regional office won’t fix it because it would come out of their pocket and Toyota won’t fix until it breaks. My research confirms this could be a bad safety issue driving around waiting for this to break. There are no codes yet but the dealership has heard it and confirmed it’s going bad and I have video of the part going bad. I always service everything with my car or home when it needs to be done and to be told to drive around until it breaks is negligent. Apparently it is a 2-3k fix. The brake assist. Driving and suddenly no brakes. Happened 2x in this vehicle and had to get a tow each time, even after getting new brake pads the first time it happened at my mechanics. The dealer confirmed today that they would fix the problem at $2700. The lights for traction and abs went off with a warning sound. Pulled over both times and called aaa for tow…..each a separate incident.

The contact owns a 2015 Toyota Prius. The contact stated while driving at various speeds, the brake, check engine, and ABS warning lights illuminated. The "Check Hybrid System - Stop Vehicle in A Safe Place" messages were displayed. The vehicle was taken to an unknown dealer where an unknown repair was completed; however, the failure recurred while driving. Additionally, the contact stated that while driving 25-45 MPH, the vehicle stalled. The contact was unable to restart the vehicle after several attempts. The contact stated that as he approached a stop light at 25 MPH, the brake pedal was depressed and traveled to the floorboard as he attempted to stop the vehicle. The brakes failed to operate without warning as he hit the vehicle in front of him. The air bags did not deploy upon impact. A police report was filed and both the contact and the other driver were able to drive away from the scene. No injuries were reported. Prior to this failure, the contact stated that the same failure occurred months prior while traveling at 20 MPH in the rain. The contact stated that the brakes failed to operate without warning as he depressed the brake pedal to the floorboard; he hit an unknown object on the road due to the failure. The contact initially thought that the failure was related to slippery roadways; however, after the most recent failure, he related the prior incident with his most recent failure. The dealer nor the manufacturer had yet to be notified of the failure. The vehicle had yet to be serviced or diagnosed. The failure mileage was approximately 80,000.

My mom was involved in an accident in her 2015 Toyota Prius on April 19, 2022, which she advised lurched forward at a stop and when applying the brakes to stop the vehicle, she said was unresponsive, causing her to clip a vehicle and sending her vehicle into a ditch totaling her car and injuring her passenger and her. I would like to have this investigated and hold Toyota responsible. This could have been much worse and lives could have been lost. She is now without means of transportation, and she is now possibly responsible for damages to property and bodily injury because of this. Through my research I have discovered multiple class action lawsuits brought against Toyota for the same exact models. I asked the police officer to run a scan on the computer, but he advised it was only for serious accidents. There was no indication of the problem or messages.

My 2015 prius gen 3 has a 118,000 miles since we bought it brand new in 2015. We periodically have it maintained with our own mechanic even after the warranty expired with the dealer. However, we experienced that the lights of Brake system, ABS and VSC went out the same time. We have it checked but there was no error code. the car is operational but the brake system does not engaged fully when you presses the brake pedal. The vehicle does not stop immediately if you need to immediately stop the vehicle. The problem strongly indicates failure in ABS control modules which can costs us a big amount of money ranging from $3,500 minimum up to $6000 max. Majority of the prius owners that shared their experiences to us regarding the same problems are unanimous in their opinion that it is indeed the brake control module. That such defect can only be pointed to the manufacturers' misdeeds.

I bought this vehicle on July 3, 2015. When I took it in for an oil change in Dec. 2015, I was informed that there was a mice infestation and that the electrical wires and other systems were damaged, resulting in about $2,800 in repairs due to the mice chewing the wiring. I just took this vehicle back in on 29 Nov. 2021 because some of the systems weren't working properly, and they confirmed that a mouse has again chewed up the electrical system, again costing about $5,000 in repairs. I've just been informed by my exterminator that Toyota uses a wiring harness coated with a soybean based coating which is causing reports of mice damage in a lot of Toyota vehicles. We have 2 other vehicles which are not Toyota vehicles and have never had a single problem with mice chewing the wiring harnesses of those vehicles, although all are parked in the same area. The vehicle is currently pending repairs at Fred Haas Toyota Country in Houston, TX and will be there for at least a couple of weeks pending availability of parts. Electrical issues of this type can cause serious failure of safety features. Toyota has never told me that they use soy based elements in their vehicles that would entice rodents to chew the wiring or otherwise entice them into the vehicle. I do not have a report from insurance yet, but I have filed a claim.

As stated from 5 months after we bought it, and the first prius, the car won't start. No matter if parked flat, keys in or out over night, or the temp, when the break is pushed it is squishy and makes draining noise and won't start. As a dr on call i have LOST patients because I could not leave my house. i carry and use the charger daily ever since. Taken in lots. they say, "you don't drive enough..or some other bogus excuse, like don't leave keys in car, which is how priuses start and run!. the car is driven in all ways a lot. also we buy headlights and headlamps on both about every 2 months. also it looses power and has shut off while driving. the worst is when it would start on its own in the locked garage. at that point my wife just quit driving all together. nothing but nightmares. She almsot crashed on fw when power went out.

On August 10, 2021 my 2015 Prius was parked in a Strip Mall parking lot. When I turned on the Prius and shifted into reverse, after traveling only a foot or so, the shift selector popped into Neutral. When I shifted into Drive, it happened again. After several tries with the same results, I called Findlay Toyota Service. I was advised to have the car towed, at my expense, to Findlay Toyota Service. Note: the tow truck driver reported to me and the Findlay Service Representative that he could keep the shift selector in gear by holding it in place with his hand while driving it to the tow truck. The Prius was left at Findlay Toyota for 2 days. The Service Rep reported that 3 different mechanics plus himself could not duplicate the problem; the Prius stayed in gear for them. I researched online and found other Prius owners reported this problem ocurring intermittantly. The only solution seems to be replacing the Shift Selector Switch at a cost of $838.35 + tax. Even though this is clearly a safety hazard, there is no recall.

While attempting to accelerate from a stop light, the Prius stalled. It would not accelerate although the engine revved. I restarted the car by powering completely off and back on. I tried to accelerate again multiple times until it slowly came up to speed the third attempt. The car was in drive and has never happened before. This is a safety concern for multiple reasons but the main being it can cause harm to those in the vehicle as well as other drivers on the road when the car stalls unexpectedly. There were no warning signals on the dash. Toyota dealer looked at it right away, ran diagnostics, and could not find an issue. Nothing was “fixed” or updated.

The contact stated that while driving at 15 MPH, the vehicle began to die as the "check hybrid system, stop the vehicle in a safe place" warning message illuminated on the instrument panel. The contact was able to barely pull off to the side and called the police for assistance. The police were able to move the vehicle to the side of the road and a tow truck was called for assistance. A police report was filed. The vehicle was initially towed to a gas station but the contact then had the vehicle towed to a dealer. While there, she discovered NHTSA Campaign Number: 20V369000 (HYBRID PROPULSION SYSTEM), which she linked to her failure; however, her VIN was not included in the recall. The manufacturer was notified of the failure. The vehicle had yet to be repaired and remained in the possession of the dealer. The failure mileage was 45,000

The smoke was first noticed by a pedestrian so I pulled over, got out of the car, and called 911. I had no prior warning of any sort. The fire completely destroyed the car. LUCKILY I WAS IN THE RIGHT LANE AND WAS ABLE TO PULL OFF ON THE SHOULDER OF THE ROAD. THE CAR COMPETELY SHUT DOWN. I'VE HAD IT TOWED TO TWO TOYOTA DEARLSHIPS, (THE SAME ONE TWICE),WHERE I WAS TOLD A NUMBER OF DIFFERENT THINGS. FIRST, I WAS TOLD THERE WAS WATER IN THE GAS TANK. THERE WAS NO WAY THAT WAS TRUE, SINCE I HAD ALREADY DRIVEN 3/4'S OF A TANK OF GAS, AND I ALWAYS FILL MY TANK UP. I REFUSED TO HAVE A NEW FUEL SYSTEM PUT IN TO THE TUNE OF $2500. THE PRIUS WAS STATIONARY, PARKED IN THE DRIVEWAY OF OUR HOUSE. MY TEENAGE DAUGHTER WAS IN THE CAR WASHING THE WINDSHIELD AND WINDOWS ON THE INSIDE AND ACCIDENTALLY BUMPED THE GEARBOX (SHIFTER). THE CAR WENT OUT OF PARK AND INTO NEUTRAL AND ROLLED BACKWARD. THE ENGINE WAS NOT ON. THE CAR QUICKLY ROLLED 20 FEET INTO A TREE AND CAUSED 6200$ DAMAGES TO THE REAR OF THE CAR. LUCKILY NO ONE WAS BEHIND THE CAR. I BELIEVE THAT THE PRIUS CAN BE ACCIDENTALLY PLACED IN NEUTRAL IF ELECTRICAL IS ON IN THE CAR (IE RADIO) AND SOMEONE HAS THEIR FOOT ON THE BRAKE AND HITS THE GEARBOX BY ACCIDENT. THERE IS NO SAFETY ON THE SHIFTER TO PREVENT UNINTENTIONAL SHIFTING. MANY OTHER CARS REQUIRE A MANUAL BUTTON ON THE SHIFTER TO BE DEPRESSED PRIOR TO SHIFTING. THIS IS NOT THE CASE ON THE PRIUS. WITHOUT THAT SAFETY FEATURE, ACCIDENTS LIKE THIS ONE CAN HAPPEN AND POTENTIALLY KILL SOMEONE. THE CAR CAN BE MADE TO BE IN NEUTRAL BY ACCIDENT RATHER EASILY.

THE BRAKES ON MY 2015 PRIUS TOTALLY FAILED AND I HAD TO SWERVE TO AVOID CRASHING INTO THE CAR STOPPED IN FRONT OF ME. I'VE BEEN DRIVING FOR MORE THAN 30 YEARS AND HAVE NEVER EXPERIENCED ANYTHING LIKE THIS. IT WAS TERRIFYING. TO WIT: I WAS DRIVING ABOUT 20 M.P.H. ON CITY STREETS IN LONG BEACH, CALIFORNIA, IN APRIL 2019. AS I DROVE OVER A BRIDGE, TRAFFIC WAS COMING TO A STOP AT A RED LIGHT 200 YARDS IN FRONT OF ME. I DROVE OVER A NARROW METAL PLATE--THERE ARE SEVERAL ON THE BRIDGE--AND HIT THE BRAKES, BUT MY CAR DIDN'T SLOW. A WARNING LIGHT FLASHED--A STABILITY CONTROL LIGHT, I BELIEVE, DUE TO THE METAL PLATE. I PRESSED THE BRAKES TO THE FLOOR AS MY CAR ROLLED DOWNHILL ON PAVEMENT. MY CAR DID NOT SLOW AT ALL. I LOOKED IN THE DRIVER SIDE MIRROR AND SAW THAT THE LANE TO MY LEFT WAS CLEAR, SO I SWERVED INTO IT WITH MY FOOT ON THE BRAKES. THE LIGHTS CANNOT BE REPLACED - THE WHOLE UNIT HAS TO BE REPLACED, WHICH COSTS ALMOST $300. THE DEALER TOLD ME THERE IS WATER IN THE UNIT. THIS CAR IS GARAGED AT WORK AND AT HOME. AS A NEW CAR OWNER I HAD NO IDEA AND IF IT HADN'T BEEN FOR MY PARTNER I WOULD HAVE NOT KNOWN. THIS POSES AS A SAFETY HAZARD AND IF THE DEALERSHIP KNOWINGLY DID NOT CHECK FOR THIS PROBLEM THEN THAT IS NEGLIGENT KNOWING THIS WAS A PROBLEM FOR THIS YEAR MAKE AND MODEL. THE MANUFACTURER WAS ALSO NOTIFIED OF THE NOTIFICATION AND PROVIDED THE SAME INFORMATION AS THE DEALER. THE MANUFACTURER PROVIDED CASE NUMBER: 1909041948. THE VEHICLE WAS NOT REPAIRED. THE CONTACT STATED THAT THE SHE BELIEVES THAT THE .DRIVER SIDE SEAT BELT AND HEAD REST FRACTURED AS HER HUSBAND WAS FOUND LAYING UNCONSCIOUS IN THE REAR PASSENGER SEAT FACE DOWN. THE SOUND OF THE ENGINE WAS LOUD ENOUGH THAT PEOPLE IN NEARBY HOUSES HEARD IT. MY CAR WOULD NOT SLOW WITH MY EFFORTS TO BRAKE. I TRIED TO SHIFT INTO NEUTRAL AND PARK BUT THE CAR DID NOT SLOW DOWN. IT RACED ACROSS THE STREET AND PLOWED DIRECTLY INTO A NEIGHBOR'S STONE WALL. MY CAR BECAME WEDGED BETWEEN THE WALL AND A LARGE TREE. I HAD TO EXIT THE VEHICLE FROM THE PASSENGER SIDE AS THE DRIVER'S DOOR WAS PRESSED AGAINST THE TREE. AFTER THE COLLISION, INTERIOR LIGHTS WERE NON-FUNCTIONING EVEN WHEN THE DOOR WAS PROPPED OPEN. THE CAR WOULD HAVE CRASHED THROUGH THE HOUSE IF IT HAD NOT BEEN STOPPED BY THE WALL AND TREE.

I WAS SITTING IN THE PASSENGER SEAT OF THE CAR WHILE IT WAS PARALLEL PARKED. I PRESSED THE PUSH TO START BUTTON TO TURN ON THE BATTERY SO I COULD CHARGE MY PHONE. THERE WAS NO ONE IN THE DRIVER SEAT. THE CAR TURNED COMPLETELY ON WHEN I REACHED OVER TO PRESS THE BUTTON. NOT ONLY DID THE ENGINE START, BUT WITHOUT ANYONE PRESSING ON THE BREAKS THE CAR RELEASED ITSELF FROM BEING PARKED. THE CAR STARTED ROLLING BACKWARDS INTO THE CAR PARKED BEHIND ME. I HAVE NO EXPLANATION ABOUT HOW THIS HAPPENED. I HAVE BEEN READING INTO IT AND APPARENTLY IT'S A DEFECT OF THE 2015 AND OLDER MODELS. WHEN THE CAR STARTED ROLLING BACK I ALSO STARTED PRESSING BOTH THE PARK AND PUSH TO START BUTTONS TO TURN IT OFF AGAIN. THE DASH LIGHTS STAYED ILLUMINATED THE REMAINDER OF THE TRIP. AFTER TAKING IT TO THE DEALER, THEY RESET THE COMPUTER AND RETURNED THE CAR. WHILE DRIVING IT HOME FROM THE DEALER, THE SAME PROBLEM REOCCURRED. THE DEALER THEN REPLACED THE MASTER CYLINDER AND CLAIMED THE PROBLEM WAS FIXED. A FEW MONTHS LATER, THE SAME PROBLEM REOCCURRED. ONCE STARTED, THE PROBLEM OCCURS EVERY TIME THE VEHICLE IS DRIVEN. AFTER DRIVING FOR A FEW MOMENTS, THE DASH LIGHTS ILLUMINATE, THE ERROR MESSAGE POPS UP, AND THE CAR UNEXPECTEDLY LURCHES WHENEVER STOPPED. WHENEVER THE HANDLE IS RELEASED, IT SPRINGS BACK TO THE CENTRAL POSITION, WHETHER THE TRANSMISSION IS IN GEAR OR NOT, SO THE DRIVER DOES NOT KNOW IF THE TRANSMISSIONS IS ACTUALLY IN GEAR OR NOT--OR IF IN GEAR, WHICH GEAR. THE DRIVER IS EXPECTED TO LOOK AT THE INSTRUMENT PANEL SO SEE WHAT GEAR IS HIGHLIGHTED IN THE DISPLAY. I RENTED A PRIUS FOR A WEEK AND NUMEROUS TIMES, I THOUGHT I HAD SHIFTED INTO DRIVE (OR REVERSE), BUT WHEN I PRESSED ON THE ACCELERATOR, NOTHING HAPPENED BECAUSE IT HAD NOT GONE INTO THE SELECTED GEAR, AND I DID NOT KNOW THAT BECAUSE THE SHIFT LEVER ALWAYS SPRINGS BACK TO CENTER POSITION, WHETHER THE TRANSMISSION IS IN GEAR OR NOT. ONCE I QUICKLY TRANSITIONED FROM THROTTLE TO FULL BRAKING AND THE CAR DIDN'T SEEM LIKE IT WAS APPLYING FULL BRAKING. IT DIDN'T SEEM TO DECELERATE LIKE IT SHOULD HAVE. I REAR ENDED A VEHICLE BECAUSE OF THE BRAKING PERFORMANCE. ANOTHER TIME I HAD TO SLAM ON THE BRAKES COMING OFF AN INTERSTATE AND THE BRAKES ALSO DIDN'T SEEM POWERFUL ENOUGH. I HAD TO CALL OUT A TOW TRUCK TO JUMPSTART THE BATTERY BOTH TIMES. CAR WAS PARKED AND DIRECTIONS WERE FOLLOWED EXACTLY FOR CHARGING IT. I TOOK IT TO THE DEALER WHO FOUND NOTHING WRONG AND WAS TOLD I SHOULD HAVE A DEDICATED LINE TO CHARGE THE CAR, EVEN THOUGH I WASN'T TOLD THIS BY THE SALESMAN. THE ELECTRIC SERVICE IN MY GARAGE IN BRAND NEW. THIS IS RIDICULOUS TO GO TO THIS EXPENSE AND NOT BE TOLD IN ADVANCE, AND TO HAVE A NEW BATTERY NEED CHARGING. THESE DISTRACTIONS MAY INCLUDE MOVING HEADS AWAY FROM THE WHEEL AREA IN ORDER TO GET A BETTER VIEW OF THE SCREEN OR ATTEMPTING TO HOLD OBJECTS ABOVE THE SCREEN IN ORDER TO MINIMIZE THE GLARE FROM THE SUNLIGHT. I WAS INCHING INTO A GROCERY STORE PARKING SPOT IN MY NEW, PERFECTLY MAINTAINED 2015 TOYOTA PRIUS WHEN I EXPERIENCED UNINTENDED ACCELERATION. MY VEHICLE SHOT FORWARD AND IN A SECOND HAD CRASHED OVER A CEMENT CURB, THROUGH A FLOWER BED, AND WAS STOPPED - DESPITE MY DESPERATELY JAMMING THE BRAKE - ONLY BY SMASHING INTO A BRICK WALL. THE SUSPENSION AND STEERING ON THE CAR IS NOW SEVERELY DAMAGED, AND THE CAR CANNOT BE DRIVEN. I AM HORRIFIED: I COULD HAVE BEEN SERIOUSLY INJURED AND WORSE, HIT/HURT ANOTHER PERSON (OR AT THE VERY LEAST SEVERELY DAMAGED PROPERTY). WHAT IF I HAD CHOSEN A PARKING SPOT IN FRONT OF A GROUP OF CHILDREN?!! THEY CLOUD HAVE BEEN KILLED! THE CAR IS NOW AT A TOYOTA SERVICE CENTER WAITING FOR TOYOTA CORPORATE TO "DECIDE" IF A CAR ACCELERATING INTO A BRICK WALL OF ITS ACCORD IS A "DESIGN FLAW" THAT IT WILL PAY TO FIX. I BELIEVE THAT WHAT MAY HAVE HAPPENED IS THAT THE COMPUTER SYSTEM THAT CONTROLS THE THROTTLE/ACCELERATION EXPERIENCED A SOFTWARE BUG WHICH THREW THE CAR FORWARD. IN FACT, TOYOTA WAS FORCED PAY OUT OVER ONE BILLION DOLLARS IN FINES LAST YEAR (2014) BY THE US GOVERNMENT FOR THIS EXACT ISSUE - BUT APPARENTLY IT DIDN'T FIX ITS 2015 LINE B/F LETTING IT GO TO MARKET. I PURCHASED THE TOYOTA PRIUS BECAUSE IT WAS RECEIVED TOP SAFETY AND RELIABILITY RATINGS. ONLY NOW DO I LEARN TOYOTA HAS A HISTORY OF HIDING UNINTENDED ACCELERATION BUGS IN ITS CAR DESIGNS FOR YEARS - AND BLAMING OWNERS FOR ITS RECKLESSLY UNSAFE AND THEREBY UNETHICAL AUTO DESIGNS (NO, THIS WASN'T CAUSED BY A STUCK FLOOR MAT). I AM FURIOUS. NO ONE CAN AFFORD TO BE BLAMED FOR VEHICULAR MANSLAUGHTER DUE TO MANUFACTURER NEGLIGENCE. EVEN IF TOYOTA CLAIMS TO HAVE FIXED MY PRIUS, ITS WORD CLEARLY MEANS NOTHING AND I DO NOT TRUST THE COMPANY. I WILL BE GETTING RID OF THE CAR ASAP AND SEARCHING FOR A SAFER MODEL.

I PULLED INTO MY DRIVEWAY TO PARK THE CAR. I PRESSED THE POWER OFF BUTTON TO TURN OFF THE CAR. THE SHIFT LEVER WAS IN THE DRIVE POSITION. THE CAR DISPLAY SAID GOODBYE BUT THE CAR DROVE FORWARD. I JAMMED ON THE BRAKE AND HIT A WAGON CAUSING $1300 DAMAGE. A CHILD OR ANIMAL COULD HAVE BEEN HIT. THE OWNER'S MANUAL STATES ON P.188 THAT IF THE HYBRID SYSTEM OF THE CAR IS TURNED OFF WITH THE SHIFT LEVER IN A POSITION OTHER THAN PARK, THE CAR WILL BE TURNED TO ACCESSORY MODE.

TL* THE CONTACT OWNED A 2015 TOYOTA PRIUS. WHILE ANOTHER DRIVER WAS OPERATING THE VEHICLE AT APPROXIMATELY 40 MPH, ANOTHER VEHICLE CRASHED INTO THE FRONT END OF THE VEHICLE. AS A RESULT, THE ROOF COLLAPSED ON THE DRIVER AND TWO PASSENGERS. THERE WERE TWO FATALITIES UPON IMPACT AND THE THIRD PASSENGER WAS AIRLIFTED TO A HOSPITAL. THE DOCTOR DIAGNOSED THAT THE THIRD PASSENGER WAS BRAIN DEAD, WHICH LED TO HER DEATH DUE TO THE ROOF COLLAPSING ON HER HEAD. THE VEHICLE WAS DESTROYED AND TOWED AWAY. A POLICE REPORT WAS FILED. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE VIN WAS UNKNOWN. THE APPROXIMATE FAILURE MILEAGE WAS 1,000. UPDATED 9/25/15 *LN UPDATED 11/28/2017*CN

I WAS PULLING OUT OF A PARKING SPACE WHERE MY FRONT WAS UP AGAINST A BUILDING WITH A CAR ON EITHER SIDE OF ME. I REVERSED FOR ABOUT 25' WHERE A TELEPHONE POLE MANDATED THAT I MAKE A K-TURN TO GET OUT. AS I SWITCHED FROM REVERSE TO FORWARD, I SAW A SPARK AS THE CAR JET-PROPELLED FORWARD. I TRIED TO BREAK MULTIPLE TIMES, BUT THE CAR ACCELERATED THE 25' INTO THE WALL. ALL AIRBAGS DEPLOYED, AND FLUID GUSHED OUT OF THE FRONT END. THE CAR WAS DECLARED A TOTAL LOSS. I WAS TOLD BY TOYOTA AND MY INSURANCE COMPANY THAT I MUST HAVE ACCIDENTALLY ACCELERATED INSTEAD OF REPEATEDLY BREAKING. I AM GRATEFUL THAT I HIT A WALL INSTEAD OF PEOPLE. SOMEONE COULD EASILY HAVE DIED HAD I HAD SWITCHED FROM REVERSE GEAR TO FORWARD IN LESS CONFINED LOCATION. THE ENTIRE ACCIDENT LASTED 2 SECONDS, WITH NO OPPORTUNITY TO PULL THE EMERGENCY BREAK OR GO INTO NEUTRAL. PS: I AM A HAPPILY MARRIED DOCTOR WITH BEAUTIFUL CHILDREN AND GRANDCHILDREN, AND NOT AT ALL SUICIDAL. THIS THREE MONTH OLD 2015 PRIUS 3 FAILED BEFORE THE FIRST OIL CHANGE. I AM NOT SATISFIED WITH THE RESULTS.

COMPLETED RIGHT TURN WHILE ACCELERATING TO ATTAIN HIGHWAY SPEED IN 45 MPH ZONE. UPON COMPLETION OF TURN DRIVER TURNED STEERING WHEEL LESS THAN 2 INCHES TO DRIVERS RIGHT TO CONTINUE BEARING ON STRAIT COURSE DOWN HIGHWAY. IMMEDIATELY THEREAFTER STEERING WHEEL SHUDDERED, AND STEERING WHEEL ROTATED HARD TO THE RIGHT FOR WHAT SEEMED LIKE A 110 PLUS DEGREE ROTATION.
